The LVN III functions with a high degree of independence under the supervision of a licensed provider or RN. This role provides advanced nursing care to patients of all ages, supporting complex patient evaluations, care coordination, and clinical operations. The LVN III applies advanced clinical knowledge to ensure safe, high-quality, patient-centered care while promoting customer satisfaction and a positive work environment.

As a successful candidate, you will:

  • Provide direct patient care in accordance with individualized care plans and under provider supervision.

  • Conduct patient intake, triage, and comprehensive assessments (subjective and objective data collection).

  • Monitor patient conditions and report changes to the healthcare team.

  • Administer medications via approved routes; perform immunizations and IV therapy (if certified).

  • Perform nursing procedures including wound care, phlebotomy, CVAD care, blood product administration (if certified), and assist with complex procedures.

  • Prepare patients for exams/procedures and assist providers during visits.

  • Educate patients and families on health conditions, treatments, and preventive care.

  • Coordinate patient care across departments to ensure continuity and patient-centered service.

  • Maintain exam rooms, equipment, infection control standards, and PHI compliance.

  • Perform EHR documentation, enter provider orders, manage test results, and serve as EHR superuser when assigned.

  • Support clinic operations including scheduling assistance, pre-certifications, clerical tasks, and optimizing patient flow.

  • Respond to clinic emergencies, activate codes, and assist with evacuations as needed.

  • Participate in quality improvement initiatives, clinical research activities (as applicable), and professional development.

  • Mentor and precept LVNs, medical assistants, and support staff.

  • May assist with oversight of daily procedure clinic operations.

Your qualifications should include:

  • Completion of a vocational nursing program

  • Minimum 3 years of experience as an LVN in a primary care clinical setting

    • Strong background in patient assessment

    • Demonstrated independent decision-making skills

  • Must meet at least two (2) of the following additional experience requirements:

    • Currently performing IV therapy as part of clinical role

    • Currently performing blood withdrawal (phlebotomy) as part of clinical role

    • Proficient in and actively utilizing care coordination

    • Leading clinic throughput and patient flow initiatives

    • Served on a quality improvement team

Required Certification/Licensure:

  • Current LVN licensure in the State of California.

  • BLS certification.

Salary / Pay Rate Information:
Pay Rate: $32.33 - $48.49 / hour

The estimated pay scale represents the typical [salary/hourly] range City of Hope reasonably expects to pay for this position, with offers determined based on several factors which may include, but not be limited to, the candidate’s experience, expertise, skills, education, job scope, training, internal equity, geography/market, etc. This pay scale is subject to change from time to time.
